IMPORTANT: 1. Request the questionnaire and the informed consent form from the laboratory. (solid tumor form) 2. The questionnaire completed by the requesting physician and the informed consent signed by the patient must be sent. Tissue biopsy - Properly labeled paraffin block (surgical specimens fixed in buffered formalin and paraffin-embedded) (FFPE - formalin-fixed paraffin-embedded). Samples must be accompanied by the pathology report for the specimen(s) to be studied and a serial section stained with hematoxylin and eosin.
